apply a curve

So all I want to do is get the attached shape to bend along the curve through the center of the form, making a smooth overall 90 degree bend in the mesh from front to back. However, no matter where I put the origin of the curve, scale the curve,orient the curve, move the mesh, whatever, I cannot get it to do so. I was able to get it how I wanted with an earlier shape, however I had deleted this shape as I had ended up changing it too drastically from what I had in mind for the shape before bending it. So basically, I know it’s possible, but I can’t recreate it for the life of me. Please help me!

If there is some other manner of achieving a bend in the mesh like described, like through other modifiers or settings, I’m all ears. It doesn’t necessarily have to be with a Bézier curve modifier.
