everything seems to be easy,and I realy don,t see there many options,and many pleace for make some error in the modifier setup.
but my results is absolutly failur:

Try Ctrl+A in object mode to reset your objects scale value back to 1. You probably rescaled your object on object mode not edit mode. This gave different scale values for each object axis. Check in the transform properties (N)
