Blender Creating its Own Vrtices...

So when I am extruding to make new faces to sculpt ill go to adjust the new and old vertices but when I move the original vertices it keeps one on the original point so two are created. Is there a way to stop that or turn it off. Any advice would help since its really difficult to model having to always delete and find the unwanted newly created vertices.

Thanks for the help!

Show us clearly what you are talking about (annotated screenshots, screencast etc etc)

If you occasionally E extrude and Esc cancel extrusion this does not delete created by pressing E vertex, face or edge. You’d need to press Ctrl-Z after Esc.
This was what could have occasionally happened during modelling.

To fix such errors it’s enough from time to time select all and W - Remove Doubles. You can adjust vertex merging threshold on T-panel after you have issued Remove Doubles according to your model vertex density: default value could be too low; if it is too high you can loose some valuable vertices.

I think Eppo is right that you have accidentally added in some extra vertices without realizing it.

Just a note on the topology you showed, you have a loop going around the mouth and across the nose, which is one of the loops you need, but you should also have one going around the lips area.
