There many different method.
You can model your clothing on top of a character, and if animated hide/delete the underlying body parts so it does not go through the clothing (i think Sintel had such a thing)
You can just extrude the cloth out of the character body, so it will deform without any mesh going through mesh, as it will all be the same mesh
You can simply texture the cloth on the body, again no problem with mesh going through mesh
Depends on the cloth, or not?
for a cape in example i would use a seperate mesh, so i can add features to the back. without geometry in my way.
for throusers i prefer to edit the caracter directly, and mudel/sculpt wrinkles and folds directly to the leg’s geometry.
