Hi, i am trying to roll up this flat looking model. But some side planes appear.
Any suggestions ?

It looks like a mess, which I think is down to a lack of geometry. You have possibly n-gons or faces that are too large, so bending the mesh means that those faces are stretched in weird ways and you end up with non-planar faces.
My suggestion would be to fix up the mesh so that you have a nice, even grid of faces (subdivided like you would for cloth simulations), which can allow a better deformation than what you have here.
I could also add that a possible alternative is to use a solidify modifier, where you only deform a subdivided plane of the shape. That way, it should reduce the amount of faces clipping through and overall not playing nice with each other.
