Check out the last selection for the last extrude, when it gets selected, there’s an overlapping edge that is not highlighted(selection does not go all the way around). What if you try a Remove Doubles first and then do it again with the original value on line 166.

You do destructive extrude on selected face in left Cube. Then the same in cube on the right (center in blender coordinates). Anyway I am not sure here about the requirements of the addon regarding transformations, object origin, pivot, etc.
Does that mean that you can not reproduce the problem with my file?
Sometimes I have problems randomly with this file too. But it behaves somewhat different from my file. Tomorrow I will analize what the differences between the two files are to see why they behave differently.

The thing is that using factory settings in blender, destructive extrude in default cube is acting more like my file, than your file. That is, I rename “2.79” folder in my user blender config folder to start with factory settings, then install destructive extrude and apply in default cube with subdivide 2 times.
Could you confirm me if with blender factory settings destructive extrude acts like it does in my file?
So then, it could be the configuration that you use making it act differently.

Seems I have found one of the problems. The thing is that the offset edge doesn’t work well with one edge. @YAFU has made an example where one edge moved. So probably this is the issue. I’ll do my best fixing it in the nearest future.
