Fluid Sim distortion on frame 1

Sometimes when I run the Fluid Sim, the first frame distorts badly.
Here’s a picture of a mesh of a figure in the fluid sim on the first frame.
Those projections jetting out the left are not there in the original mesh.


Does anyone know why the Fluid Sim distorts things this way?

Before baking any fluids I always make sure that all objects scale and rotation has been cleared (Ctrl+A).

I tried clearing scale and rotation, but that doesn’t help.

I’ve done this with a simple mesh before, but it seems if you have a mesh that is more complicated, the simulator messes up. Has anyone else noticed this?
