Help with deformed parented object

I would like to parent an small object to a larger object in the game engine
However, when I press P, the smaller(parented) object changes shape
Can someone explain this and how do I get around it?

I’ve attached pictures of a test, the first shows the 3D view, the second, the small box is deformed when P pressed.
