How do I achieve this cupcake icing?

I see two possibilities, both uses curves as central element:

  1. with only curve
  • just make a spirally curve. Then under the curve properties > geometry > bevel add some depth to it et voila!
  • to make it thinner at the and you need another curve that defines you taper/profile and link it under geometry > taper object of the spiral.
  1. a long tube with a curve modifier:
  • again spirally curve
  • for geometry add a long subdivided cylinder (or whatever) and align its origin exactly with that of the curve.
  • add a curve modifier to the cylinder referencing the spiral.

To melt it all together I would add a remesh modifier on top
