How Do I Fix Holes in Sub-surf Mesh?

I have a few holes in my sub-surf mesh where the creases I added don’t want to hold the corners in tight. Is there any trick to deal with this, or should I be relying more on loop cuts, etc., to model? I’m not concerned about poly count.

I think you simply have double vertices there. I may be wrong though.
