Mesh shreds apart after leaving sculpt mode to object mode randomly

This has been happening to me recently in new projects where after leaving sculpt mode the mesh becomes shredded. In edit mode all the verts are all over the place. Trying undo/redo does not resolve problem. This has happened in version in screen shot and previous version before updating to this one. I am about to update to latest build but want to know if this is a know issue or if I am doing something wrong.

Using latest version of Mojave on macbook pro.

So far I have been using Blender 2.8 build: 6/9/19 and I have not experienced this issue. Perhaps it was a bug with the previous builds I was using.
