non-connecting vertices after loopcuts

Can someone explain to me why this is happening (am I doing something wrong or is this a bug?) I am using blender 2.67b
I created a plane, went into edit mode, stretched the plane along the x-axis (by grabbing each side), then I made a loop cut along the y-axis and scaled it into position, and then I created a loop cut and scaled it along the x-axis. giving me the first image.
Then I noticed that selecting the center most face was impossible, so I checked the topology and noticed that the edges were arranged in a sort of z-shape along the z-axis (see image 2 and 3 where I pulled the vertices out a bit to show the arrangement).

  1. How did this happen,
  2. how do I prevent this, or how do I solve this?

Are you 100% sure this is all you did ? Did you duplicate the plane or extruded by mistake.
If you actually did only what you said you would not have a problem so I can only assume you unknowingly didn’t.

I tried it several times, making sure I repeated my steps exactly, so I am sure (as far as I’m not an idiot) that is all I did.
HOWEVER, that was yesterday; today when I tried it again (in order to make a screencast), it no longer happened. So either I am an idiot and did something stupid 20 odd times in a row, or it was some kind of cosmic radiation ~:)
