What happened?

By the way, the second image is in Play mode

Several things could have happened. If you dont select UVs>layout clipped to image size, then you can inadvertantly move the map off of the image texture in the uv window with that result. Even a slight misalignment will cause the image to tile. or you could have off set the image in the materials buttons on the map input pannel. Just select UVs>layout clipped to image - size in uv/image editor window, then unwrap>reset 1/1 and click on “Tex Face” on the Material tab in the materials buttons. Not sure really why you unwrapped a plane though.
