Here is the model I tried to rigify, but I followed several methods but they only put a few bones in the back of the model and do not generate.mod rigify.002.blend (1010 KB)
What i did :

I used Blender 2.66a

I delete the Subdivision Surface and the Armature Modifier that are on your model

I deleted every Vertex Group entries for the model

I added a new Human meta-rig from the Rigify
-
Rescaled it in Edit Mode

Using your meta rig as a guide, i edited the one i added so the bones location and size would match yours

I deleted your meta rig
Attached the blend to this post, now the Rigify Generate button works correctly and does not generate an error or does not create bizarre bones.
I have no idea what was wrong with your version , were you using an older version of Blender instead of latest 2.66a ?
It’s possible that then the old version you’re using had a bug in the Rigify meta rig that has been fixed in 2.66a.
