Smoothing groups are a nasty Max convention they’re not really groups, but outer edges that have their vertices split.
In Editing (F9) -> Modifiers, add the EdgeSplit modifier. By default, it will have From Edge Angle on, which you probably want turned off. Now you can select edges and press Ctrl+E -> Mark Sharp (not to be confused with Mark Seam).
I often simply make multiple material IDs. Then I can select only the faces associated with a particular material (ie the lips). Once you have the selection just click the smooth button.
