Assuming you want the bone in the same armature, you need to be in Edit mode to add bones. Then you can add a bone wherever you want by pressing Space Add>>Bone. The new bone will show up at the cursor and you can move it wherever you like. You can connect it to the previous bone by selecting the bone you want to be the child, then in the Edit buttosn Armature Bones tab selecting the parent bone name in the Child Of drop-down menu. If you want the bones connected click the “Con” button to the right of that drop-down.
