It is just this computer I am using now and 2.49 forward as far as I can tell to 2.79.
It is a PC workstation with an Nvidia card.
Any idea what would make this shortcut fail?

Control+R for loop cutting? It’ll only work on quads. not faces with 3, or 5 vertices. If thats not the problem and your geometry is fine. Go to user preferences > input and check if somehow you accidentally changed the key binding for loop cuts.
A fast way to check all shortcuts for a given key is the “Is Key Free” plugin.
I checked it to be sure. But no it would not be key mapping as I said it happens in 2.49 as well. Also 2.78 (separate build from another location that does not share preferences) And it is only on this particular machine.
Any other reason why a key mapping would simply stop working?
The tool works correctly from the Tools Tab.
Could be something in your OS or another program that is intercepting the keystroke. Some programs are really dumb about this. There is a program on my work computer that intercepts (CTRL-SHIFT-S) so it can take screenshots. Madness.
