is there an automatic/semiautomatic way to generate it, or do I need to model it all by hand?

Ideally you would have a b+W height map but just using your original image in the displacement modifier gives
The model above is real geometry but if you just want to fake it with as a bump map it would give

I wonder if I feed it a transparent png, the geometry will have holes. Thats what i want with my real medallion. I’ll try and see.

No it won’t. All it would mean is there will be no displacement of the mesh
