Did you sculpt the relief? Or have you figured out a really good image-to-displacement mapping? Either way, this looks very nice. 
Thank you dear shannonz, It was fully sculpted but I use the picture as a reference behind the work but not as a displacement map . It means that I can make this also with physical clay or clay dough exactly like this, which I had been doing this for years before entering the 3d world.
